- Online at www.ssa.gov; OR
- Through the local Social Security Administration Office (click here to determine the closest Social Security office nearest you: https://secure.ssa.gov/ICON/main.jsp)
Regardless of which way you apply, you will need the following information:
- List of medical providers with addresses and phone numbers;
- Dates of medical treatment (including doctor visits, hospitalizations, lab work, and medical tests);
- Personal information, including dates of prior marriages; AND
- A detailed work history for the past 15 years, including the names, addresses, phone numbers, and dates of employment for each job.
